GROUP LIFE/AD&D Valuable group life insurance coverage for groups
of ten or more employees. Competive guaranteed issue limits; up to
$500,000 based on group size. Dependent life for spouse and children
is available.
GROUP VOLUNTARY LIFE
Voluntary coverage for employees, spouses, and
dependent children. A level of guaranteed issue coverage is available
for employees. Employee AD&D coverage is available. Life coverage
is portable in most states.
GROUP VOLUNTARY SHORT & LONG
TERM DISABILITY
Employer sponsored and employee paid plans provide
50 percent or 60 percent replacement of income due to illness or
injury up to liberal weekly and monthly maximums.
WEEKLY DISABILITY INCOME
Protects a selected percentage of the employee's
basic wages for an established period of time in the event
the employee is unable to work due to illness or injury. Benefits
available according to group size.
GROUP LONG-TERM DISABILITY
Provides employees with financial assistance
when income loss results from a serious and prolonged disability.
Choice of benefit percentage up to a maximum of $10,000 monthly.
Choice of several elimination periods available. Benefits include
partial disability and survivor benefit. Many available options
to meet the needs of most employees.

SELECT FOR SMALL FIRMS (2-24 Employees)
Employers Group Benefits Trust, underwritten
by Security Mutual Life Insurance Company of New York and administered
by Managing Agency Group, will provide your small-employer
clients with high-quality insured benefits designed for firms
of two to twenty-four employees. Life, AD&D, Dependent
Life, Short and Long-Term Disability coverages are available
to most firms.
Statutory Disability
Benefits:
Benefits conform to the then-current state-mandated
benefits.
Benefits are payable for a maximum of 26
weeks (within a consecutive 52 week period).
Benefits commence on the eighth consecutive
day of disability.
Benefits are paid retroactive to the first
day if disabled for three weeks or more.
NEW YORK STATE DISABILITY BENEFITS
LAW (DBL)
Provides cash payments to employees
for replacement of lost wages due to illness or injury
occurring off the job. Security
Mutual's DBL plan provides risk-bearing protection
to the employers and financial responsibility to the employees
in the event an off-the-job disability results in lost
wages.
STATE OF NEW
JERSEY TEMPORARY DISABILITY BENEFITS (TDB)
TDB Provides cash payments to employees
for replacement of lost wages due to illness or injury
occurring off the job. Security
Mutual's DBL plan provides risk-bearing protection
to the employers and financial responsibility to the employees
in the event an off-the-job disability results in lost
wages.

Underwritten by Security Mutual and administered
by Granite Capital Holdings, Inc., VESO is group life coverage
for active members of volunteer fire and volunteer emergency
